[SDL] [maybe patch] allow fullscreened apps on head "0" with xinerama

this patch by Diego Petteno <flameeyes at gentoo.org> allows you to set 
SDL_VIDEO_FULLSCREEN_HEAD to 0 to indicate you want to use just head "0".  
right now SDL will treat a value as "0" as meaning "unset".

i'm not sure this patch is still needed as i'm not familiar with the xinerama 
code, but a quick glance makes me think it is ...
